Who is Covenant Care?

Covenant Care offers comfort and care for patients at many key stages of their lives. A wide range of services, including hospice, palliative care and home care, with specialized programs for children, veterans and those facing Alzheimer’s disease, Covenant empowers patients and their loved ones with more choices, allowing them to live each day to the fullest. Covenant Care’s commitment extends to caregivers and family members who work tirelessly to care for their loved one. Whether it’s counseling, support groups, home visits or simply a hand to hold during turbulent times, Covenant embraces the entire family with compassionate care. Our collaborative team of specialists includes physicians, spiritual care coordinators, nurses, social workers, bereavement specialists, personal care aides, volunteers and therapists. Covenant’s reach extends to 50 counties in Northwest Florida and South Alabama. Through numerous partnerships with hospitals, nursing facilities, assisted living communities and other organizations throughout the service area, Covenant offers care in a variety of settings when living at home is not possible.

What Does Covenant Care Offer?

Covenant Care’s integrated approach allows patients to easily access numerous services and programs along the continuum of care.

Hospice: We are widely recognized as one of the largest and most comprehensive hospice programs in the nation, providing a range of services to patients with life-limiting illnesses. Through our Open Access hospice program, Covenant offers patients the benefits of hospice services while they continue to receive many treatments and medications that are not typically offered within a traditional hospice program.

Palliative Care: Palliative Care services can provide relief from pain and alleviate distressing symptoms. Patients can receive palliative care any time during the diagnosis and treatment of many long-term conditions.

Care Choices: This innovative Medicare demonstration project allows patients with specific diagnoses – Congestive Heart Failure, Advanced Cancers, COPD and HIV/AIDS – the option to elect to receive comfort care services typically provided by hospice, while continuing to seek curative care and medical treatments.

Home Care: Whether a patient is recovering from a surgery, transitioning home after a hospital stay, or simply needs a bit of extra help to maintain one’s independence, Home Care offers a range of solutions to ensure patients are safe and comfortable at home.

Alzheimer’s Care: Covenant Alzheimer’s Care improves awareness and understanding of Alzheimer’s disease while supporting the needs of patients, caregivers and families faced with this and other forms of dementia, through counseling, education and direct care provisions.
