Home Health Aide
Working under the supervision of a Registered Nurse, Home Health Aides assist with the activities of daily living such as bathing, skin care, and oral hygiene for homebound clients already receiving our skilled services.
Medical Social Services
Our Medical Social Workers provide counseling to clients and/or family members to help with social and emotional stressors related to the client’s health challenge. Our specialists help family members properly care for their loved one and educate them on available community resources.
Physical Therapy
Physical Therapy will gently transition the client back to independence in daily living activities. Our dependable therapists will regularly monitor the progress of the care plan and determine, based on the review, the next course of treatment to ensure that our client gets the best care possible.
Occupational Therapy
Occupational Therapy services encourage and motivate clients to participate in various activities of daily living. The primary focus is for clients to regain independence and return to their prior level of functioning. An initial home evaluation and individualized treatment plan will ensure the client overcomes limitations and sustains the maximum level of functional ability.
Speech Therapy
Communicating your thoughts to the people around you is an important part of living. We understand that every client is unique and develop individualized plans of care.

Transitional Care
Following hospital discharge, the Transitional Care Health Coach will be in direct contact with the physician(s) and will be available to accompany clients during their first follow-up visit with their physician. Steady Steppers

VitalStim
VitalStim Therapy is a therapeutic intervention for use in the treatment of dysphagia (swallowing disorders). It is a specialized form of Neuromuscular Electrical Stimulation (NMES), specifically designed to treat dysphagia.
